Required Qualifications: Education to meet certification, license or registration requirement Upon hire: Oregon Licensed Practical Nurse Within 30 days of hire: National Provider BLS - American Heart Association Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. Notes: (1) License in states wherever care is delivered 1 year of experience in Acute Care, Home Health, public health nursing, or patient care experience in a skilled nursing facility or medical clinic.
