This Home Health Occupational Therapist position is based in Kirkland, Washington, a lakeside community that blends small-city charm with the amenities of a major metropolitan region. The city's location on the Eastside provides easy commuting to Seattle and Bellevue while maintaining a relaxed, neighborhood-focused atmosphere.In this role, you will provide one-on-one, patient-centered Occupational Therapy services in the home setting for adults and older adults who are recovering from illness, injury, or surgery, or managing chronic conditions. You will travel to patient homes throughout Kirkland and nearby communities, using your clinical skills to help individuals regain independence and safely perform everyday activities in their own environment. This setting offers a meaningful opportunity to see the direct impact of your interventions on a patient's daily life, with a focus on functional outcomes, safety, and quality of life.Typical responsibilities include conducting comprehensive occupational therapy evaluations in the home, identifying functional limitations and environmental barriers, and developing individualized treatment plans. You will implement interventions such as ADL training, energy conservation techniques, balance and coordination exercises, home safety modifications, and equipment recommendations. You will provide patient and caregiver education, coordinate services with nursing, physical therapy, speech therapy, home health aides, and medical social work, and contribute to overall interdisciplinary care planning to support safe transitions and reduce rehospitalization risk.Your day-to-day work will involve managing a field-based caseload, traveling between homes, completing timely EMR documentation, and communicating with the broader care team. Visit volume and patient ratios are designed to support thorough, high-quality care and appropriate time for assessment, treatment, and documentation. You will work within established home health guidelines and regulatory standards, ensuring accurate OASIS or other required assessments, and adhering to infection control and safety protocols in diverse home environments.Shifts typically follow daytime hours with some flexibility to support patient needs and route efficiency. The role allows for a degree of autonomy and independence, while still providing support from clinical leaders and colleagues experienced in home health. Required Qualifications

Occupational Therapist, Home Health Experience: 1 year Licenses: OT-WA Driver's License and Auto Insurance
