Care Coordinator - Children's Long-Term Support (CLTS) Program Location: Waukesha County, WI Job Type: Full-Time | M-F, First Shift | Remote Flexibility Pay: $24.50/hr Sign-On Bonus: $1000 Lutheran Social Services of Wisconsin & Upper Michigan is seeking a compassionate and organized Care Coordinator to join our CLTS Waiver Program team in Waukesha County ! The Children's Long-Term Support (CLTS) Waiver Program is a federally funded Medicaid initiative that helps children with developmental, physical, or severe emotional disabilities receive services that allow them to thrive in home and community settings. As a Care Coordinator, you'll be the primary point of contact for families, helping them access and navigate essential services. Please note: This is a continuous posting in anticipation of future growth opportunities in the Waukesha County area. Qualifications Bachelor's degree in a human services field (e.g., Social Work, Psychology, Special Education, Counseling, etc.) Minimum 1 year of experience working with children with disabilities Bilingual fluency in Spanish is preferred but not required Proficient in computer systems and electronic health records Strong interpersonal and organizational skills Valid driver's license and reliable transportation (MVR check required) Work Environment Community-based with daily travel required Moderate noise level; occasional exposure to outdoor conditions Flexibility to meet family needs, including crisis response LSS is an Equal Opportunity Employer (EOE).
