Case Managers/Skills Trainers help stabilize and/or improve social and behavioral functioning in patients, as well as help ensure smooth transitions throughout the continuum of care and reintegration into the community. Case Managers/Skills Trainers also help patients connect with community partners and resources, develop strong personal support systems, as well as teach patients healthy skills that promote the recovery process.
