Veteran Health Care Case Manager & Care Coordinator
Community Bridges
Community Bridges is seeking a Health Care Case Manager for the Supportive Services for Veteran Families (SSVF) program in Phoenix, Arizona. The successful candidate will act as a liaison between the SSVF Grantee and the VA, assisting veterans in accessing health care services and adhering to care plans. This role requires strong case management skills, experience with complex needs, and sensitivity to cultural factors. Benefits include medical, dental, wellness programs, and generous PTO.
