BreakThrough Physical Therapy is seeking a dedicated and patient-centered Physical Therapist to join our Healthcare & Medical Services team. As a key member of our clinical staff, you will provide evidence-based, individualized care to patients with a wide range of musculoskeletal, neurological, and movement-related conditions. You will work in an outpatient clinic setting, helping patients improve mobility, reduce pain, restore function, and enhance their overall quality of life.

In this role, you will perform comprehensive evaluations, develop and implement personalized plans of care, and deliver hands-on interventions and therapeutic exercise programs. You will utilize best-practice treatment approaches, including manual therapy, therapeutic exercise, neuromuscular re-education, balance and gait training, and patient education. You will document all care accurately and efficiently in our electronic medical record system, track patient progress using objective outcome measures, and adjust treatment plans based on clinical findings and patient goals.

You will collaborate closely with a dynamic, interdisciplinary team of clinicians and support staff who share a commitment to excellence, innovation, and patient-centered care. You will communicate regularly with referring providers and other healthcare professionals to coordinate care and ensure optimal outcomes.
