Transitional Care Coordinator

POSITION SUMMARY

The Transitional Care Coordinator supports clients throughout Phase II, Intensive Outpatient Program (IOP), Aftercare, and alumni services by coordinating care, monitoring progress, connecting individuals to community resources, and promoting long-term recovery success. This role serves as a key resource for clients and alumni, helping individuals access recovery support, housing, education, employment, and productive living opportunities.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Essential functions include but are not limited to the following:

• Monitor client compliance, participation, and progress within Phase II, IOP, and Aftercare programs.
• Maintain client records, rosters, reports, spreadsheets, billing documentation, and other required program data.
• Distribute daily reports regarding client progress, admissions, discharges, and other program updates.
• Develop and maintain relationships with community partners and service providers to support client needs and successful transitions.
• Conduct case planning and provide ongoing guidance, referrals, and resource coordination for clients.
• Coordinate Life Skills programming, including scheduling presenters and managing related activities.
• Facilitate support groups, house meetings, and other recovery-focused activities as assigned.
• Serve as a liaison for clients involved with legal, criminal justice, and other external systems.
• Prepare completion letters and other required client documentation.
• Participate in case reviews and collaborate with treatment providers and internal teams to support client success.
• Oversee the GED program, including participant engagement, scheduling, tracking, and reporting.
• Maintain alumni engagement records and assist with ongoing alumni outreach efforts.
• Share recovery, educational, employment, and community resources with clients and alumni.
• Promote Brady Center for Productive Living services and opportunities to current clients and alumni.
• Assist with referrals and connections to Brady Center for Productive Living housing and supportive services.
• Support alumni engagement and career readiness initiatives in collaboration with internal teams and community partners.
• Works in collaboration with data department to maintain alumni database
• Perform other duties as assigned.

QUALIFICATIONS / REQUIREMENTS

• High School Diploma or equivalent required.
• Registered Peer Support Specialist (RPPS) or ability to obtain within 3 months from hire date
• Excellent written, verbal and interpersonal communication skills
• Proficiency in Microsoft Office Suite including Outlook, Word and Excel
• Strong attention to detail
• Ability to maintain confidentiality of information, people and processes
