Therapist - Physical Therapist

Physical Therapist Position

Seeking a dedicated Physical Therapist to provide home health therapy services. The role involves managing patient care visits, documentation, and travel within designated territory to support patient recovery and independence.

Responsibilities:

Evaluate patient needs and develop personalized treatment plans for adult home health patients.
Perform home visits, including Start of Care and Follow-Ups, ensuring quality and compliance standards.
Document clinical assessments and treatment progress promptly within EMR systems.
Collaborate with healthcare teams and coordinate patient care across multiple locations.
Adhere to travel schedules, covering up to 75 miles from the care center, and ensure timely visits.
Maintain current licensure, CPR, BLS certifications, and proficiency in EMR platforms.

Qualifications:

Hold a valid state license as a Physical Therapist, in hand before start date.
Possess at least 1 year of recent adult home health experience.
Demonstrate proficiency with EMR systems such as HomeCare HomeBase and PointCare.
Have reliable transportation, valid driver's license, and agency-required liability insurance.
Ability to travel up to 75 miles and accept patients regardless of distance.
Willing to work during weekdays, with flexibility for weekend and on-call rotations as needed.
