Speech Language Pathologist (SLP)

Role: The Speech Language Pathologist (SLP) provides direct speech therapy rehabilitation care to patients in a skilled nursing facility setting. Key Duties: Deliver quality speech therapy, support patient progress, ensure compliance, and contribute to a positive, patient-centered environment. Responsibilities Assess, establish, and modify functional speech therapy goals. Implement and follow individualized treatment plans in accordance with regulations and standards. Chart progress accurately and timely, utilizing therapy software. Collaborate with interdisciplinary teams, patients, and families to meet patient needs. Supervise and evaluate Speech Language Pathology Clinical Fellows (CCCs) if applicable. Participate in community health projects and uphold patient safety, rights, and confidentiality. Maintain professional relationships and adhere to company policies and legal guidelines. Assist with patient evacuation and ensure dependable attendance. Qualifications & Skills Graduate of an accredited speech therapy program (MS or MA). ASHA certification (CCC) or eligible for Clinical Fellowship Year (CFY); must maintain credentials. Current state license in good standing; CPR certification required. Preferred: 1+ year experience; new grads welcome. Proficient in Microsoft Word, Excel, and email communication. Strong decision-making, body mechanics, and teamwork skills. Knowledge of post-acute care regulations and procedures.
