Position Title:

Children's Mental Health Case Manager

Compensation (Pay/Benefits) Our employees are our most valuable asset; therefore, we offer competitive pay and benefits. The pay range for these positions starts at $50,300 - $60,300 a year for full time, depending on qualifications (i.e., licensure and experience). This salary reflects a 40-hour workweek, annualized. Any agreed-upon reduction in weekly hours will be reflected in salary commensurately. Please inquire with interest. This position is benefits eligible, including health and dental insurance, health savings account with an employer contribution, flexible spending accounts, and retirement account with matching contribution. Schedule: 40 hours per week, M-F. This position requires flexibility to work past 5pm two evenings per week.

Location: Minneapolis-Nicollet Center site with hybrid officing and most in person work being community based

Program Summary:

St. David's specializes in early childhood mental health and serves children and families with developmental delays and disabilities, physical challenges, medical complications, autism spectrum disorder, attention deficit disorders and emotional/behavioral disturbances. Our Mental Health Services integrate with all the programs offered at St. David's so that we can provide each family with the most comprehensive support.

Position Overview:

St. David's offers a unique opportunity for case managers to provide community-based support and advocacy for families who have a child with significant mental health needs. The case manager and family together assess a child's needs and develop a care plan. Case managers assist the family with locating, accessing and coordinating services or community-based supports. Case managers advocate for and assist with maintaining services with the goal of helping the child be their best at home, in school and in the community.

Responsibilities include:
• Developing supportive relationships with the child/youth, the family, and the team serving the child/youth, while consistently maintaining professional boundaries and reflective practice
• Assisting child/youth in gaining access to needed medical, social, educational, and other services to address their mental health needs while focusing on the strengths of the youth and family, viewing their needs holistically, and recommending services that will help them reach or maintain their optimal level of functioning in a community-based setting
• Meeting in the family home or community with the child/youth and their guardian(s) a minimum of once a month and completing all related documentation in a professional and timely manner

Job Qualifications:

*A Driver's license and reliable vehicle are required for this role. *

Education: At a minimum, a Bachelor's degree in one of the behavioral sciences or related fields from an accredited college or university

Experience:
• Providing mental health services to and identifying and assessing the needs of diverse children/youth with mental health diagnoses
• Familiarity with electronic health records, Word, Excel and basic computer literacy
• Strong relational and organizational skills for tracking and completing documentation

Preferred:
• At least 2,000 hours of supervised experience in the delivery of mental health services to children/youth
• Experience working independently in community settings
