Responsibilities

- Collaborate with interdisciplinary team members to support discharge planning, safety, and quality of life.
- Maintain accurate documentation in appropriate records and ensure compliance with regulatory and facility standards.
- Communicate clearly with residents, families, and team members to support goals and care continuity.

Requirements
- Active Occupational Therapist license (state-specific) eligible for CT assignments
- Ability to work in a long-term care environment and manage a caseload in a travel setting
- Strong assessment, treatment planning, and documentation skills
