Schedule: M-F 8am-5pm#AC-PTCOOffer Based on Years of ExperienceWhat You Need to KnowThe Physical Therapist is responsible for providing evaluation and treatment to patients, functioning as a member of the interdisciplinary team, providing patient and family education and counseling regarding concerns, planning, implementing and modifying treatment procedures, maintaining appropriate documentation according to policies and procedures and current standards of practice. This position manages a caseload in accordance with agency productivity standards, attends and participates in patient and other conferences and agency in-services as assigned.Be the Best Physical Therapist You Can BeIf you meet these qualifications, we would love to meet you:Master's degree in physical therapy approved by CAPTE accredited physical therapist education programMinimum nine months experience as a physical therapistPreferred community/home health experienceCurrently licensed to practice as a physical therapist within the state of agency operation and compliant with accepted professional standards and practicesMust possess and maintain valid CPR certification while employed in a clinical roleMust be a licensed driver who can travel to all business locationsResponsibilities:As a Physical Therapist, you will:Provides patient care and documents treatment in accordance with physician orders, agency policy and patient needs.Performs patient evaluations in accordance with agency policy and procedure.
