Viva Life Home Health & Home Care is seeking a compassionate and dedicated PRN Medical Social Worker (MSW) to join our growing home health team. Position Summary

The Medical Social Worker provides social work services to home health patients in accordance with physician orders, agency policies, and applicable regulations. The ideal candidate is compassionate, resourceful, and skilled at helping patients and families address social, emotional, and environmental factors that may affect their health and recovery.
Responsibilities

• Assess patients' psychosocial status and identify barriers to care.
• Develop and implement individualized plans to address social, emotional, and environmental needs.
• Assist patients and families with accessing community resources and support services.
• Provide counseling and emotional support related to illness, disability, adjustment, grief, and caregiving challenges.
• Collaborate with physicians, nurses, therapists, caregivers, and other members of the interdisciplinary team.
• Participate in care planning and case conferences.
• Document assessments, interventions, and patient progress in accordance with agency standards and regulatory requirements.
• Educate patients and families regarding available resources, advance directives, and long-term planning options.
• Promote patient rights, dignity, and self-determination.

Qualifications

• Master's Degree in Social Work (MSW) from an accredited school of social work.
• Current Texas licensure as required by state regulations.
• Minimum of one (1) year of social work experience in a healthcare setting preferred.
• Home health experience preferred but not required.
• Knowledge of community resources and healthcare systems within the San Antonio area.
• Strong communication, assessment, and documentation skills.
• Ability to work independently while collaborating effectively with an interdisciplinary team.
• Reliable transportation and current driver's license.
