RN Care Manager: LTSS & Waiver Services

Molina Healthcare is seeking a Care Manager RN to support care management and coordination for LTSS programs in Wisconsin. You will collaborate with an interdisciplinary team to assess member needs, develop and monitor care plans, and coordinate waiver services, ensuring quality and cost-effective outcomes. Travel up to 25-40% locally may be required based on state contracts and program needs; you will educate members and caregivers using motivational interviewing and Molina guidelines to #J-18808-Ljbffr Molina Healthcare
