PHYSICAL THERAPIST

Liberty Health seeks a Physical Therapist to deliver person-centered rehabilitation services within innovative behavioral health and human services programs. The PT will evaluate functional mobility, develop evidence-based treatment plans, and provide individualized interventions to support independence and community integration. Responsibilities include collaborating with interdisciplinary teams, documenting in EHR systems, educating patients and caregivers, and tracking functional outcomes. This role supports public-sector partners with data-driven, outcomes-focused care across diverse populations in a collaborative, accountable culture.
