Rapid Rehousing Case Manager — Housing Stabilization Advocate
Colorado Coalition for the Homeless
Colorado Coalition for the Homeless is seeking a Case Manager to provide coordinated stabilization and time-limited case management to clients exiting unsheltered homelessness or transitional shelter sites in Denver, CO. You will work with a collaborative team including behavioral health navigators, housing navigators and other service providers to ensure rapid exits into housing and follow-up care for at least the first year. #J-18808-Ljbffr Colorado Coalition for the Homeless
