- Collaborative Environment: Work with an interdisciplinary team to build detailed and individualized care plans.
- Conduct and coordinate Resident Assessment Instruments (RAIs) per federal, state, and local regulations.
- Collaborate with the Interdisciplinary Care Plan Team to develop resident-specific care plans.
- Evaluate resident conditions and ensure all special assessments are completed as required.
- Provide data for the Facility's Quality Assurance Program.
- Active and current LPN or RN license in the state of Missouri
- Previous experience as an MDS Coordinator
- Strong understanding of Medicare/Medicaid regulations and benefit guidelines
