Position Summary The primary function of the role is to return patients to optimal physical function and to facilitate achievement of patient, physician, and therapy goals within a SNF setting. Instruct patients on maintaining function after discharge through education, independent exercise programs, and appropriate activity modification. Education/Qualifications Bachelor’s degree or higher in Physical Therapy from an accredited college or university. Active, unencumbered license in the state of employment. Basic computer skills and ability to navigate service delivery software and electronic health record software.
