We re seeking a compassionate and motivated Nurse Practitioner (FNP) to provide high-quality care in an outpatient community health setting.

What You ll Be Doing:
- Conduct comprehensive patient evaluations, including physical exams and health assessments
- Collaborate with physicians and specialists to ensure coordinated, well-rounded care
- Create and manage individualized treatment plans tailored to patient needs
- Prescribe medications and monitor treatment effectiveness
- Support patients with both primary care and behavioral health concerns
- Coordinate referrals to specialty providers and external services
- Contribute to the development and improvement of clinical care processes
Requirements:
- Active New York State license as a Family Nurse Practitioner (FNP)
- Master s degree in Nursing
- Minimum of 3 years of experience in a primary care or outpatient setting
- Bilingual English/Spanish is a plus
