MedCap Health in Warren, OH seeks a part-time LongTerm and Post-Acute Care APP/NP/PA to reshape post-acute care. Your schedule is Monday through Friday day shifts with no overnights or weekends, and you’ll collaborate with a multidisciplinary team to manage patients with acute and chronic diseases.

The ideal practitioner is a board-certified geriatric, family, or adult NP (ANCC, ACHPN, or AANP) with 3–5 years of RN or post-acute pulmonary experience and 1+ year in post-acute or long-term care.
