Arrowhead Regional Medical Center (ARMC) is recruiting for Registered Nurse (RN) Care Managers. As part of the Ambulatory team, the RN Care Manager will work at the top of their scope of practice with chronically ill patients who possess multiple co-morbidities with high hospital recidivism to high-risk patients without utilization issues needing education in health literacy and disease management. The RN Care Manager will be responsible for educating their panel of patients on individual disease processes, how to recognize warning signs and effectively control them at home or know when to seek help. The RN Care Manager will also provide patients with the tools for self-management, confidence to navigate the healthcare system and direct access for chronic or acute illness management via face to face, telephonic, electronic media and tandem visits with their physicians. CONDITIONS OF EMPLOYMENT Pre-Employment Process: Applicants selected for these positions must pass a background investigation, physical and verification of employment history and education. License/Certifications: RN license, and Basic Life Support, and depending on the area of assignment, additional certification(s) may be required and must be maintained throughout employment. Shift Availability: ARMC is a twenty-four hour facility; applicants must be available to work all shifts, weekends and holidays. Shift differentials are provided. Candidates must possess all the following: LICENSE: Must possess and maintain a current license as a Registered Nurse with the State of California Board of Registered Nursing. CERTIFICATION: Must possess and maintain current Healthcare Provider level Basic Life Support (BLS) certification. EXPERIENCE: Option 1: Three (3) years of registered nurse experience in an acute care hospital. Option 2: Two (2) years of registered nurse experience in Primary Care, Outpatient Care, Care Management, or Community Health. Candidates possessing a Bachelor's or Master's degree in Nursing, Healthcare Management or related field are desired. Bilingual skills are highly desirable.
