The University of Miami UHealth – (department/division) at SoLé Mia has an exciting opportunity for a Patient Navigator The University of Miami UHealth at SoLé Mia has an exciting opportunity for a Patient Navigator to work full time on site. The Patient Navigator (H) facilitates patients’ ability to obtain services by identifying barriers to care, coordinating the development of treatment plans, and monitoring the implementation and follow-up of services ordered by caregivers.

CORE JOB FUNCTIONS

Conducts patient evaluations to obtain information necessary for the development of an effective treatment plan. Ensures patients have access to insurance and social service programs for which they are eligible. Provides patient reminders of upcoming appointments and acts as liaison between patients and providers. Identifies and resolves real or perceived barriers to care. Educates and counsels patients on issues related to their health and life situations. Maintains current knowledge of community health, welfare resources, and any other programs available to patients. Handle patient communications and contact healthcare facilities for medical records. Address care gaps with the nurse navigator. Prepares, tracks, and processes medical records and pathology specimens. Reviews and file medical records for completeness; use tracking systems for accurate documentation. Provides administrative support to the department by answering phones, address inquiries, and direct calls or take messages, Copy, fax, emailing, and mailing records while ensuring confidentiality. Adheres to University and unit-level policies and procedures and safeguards University assets. CORE QUALIFICATIONS

High School diploma or equivalent Minimum 1 year of relevant work experience Knowledge, Skills and Attitudes: Ability to process and handle confidential information with discretion. Ability to communicate effectively in both oral and written form. Ability to handle difficult and stressful situations with professional composure. Ability to maintain effective interpersonal relationships. Ability to understand and follow instructions. Department Specific Functions • On-Site, 5 days per week.• Maintains the Welcome/Patient Resource Center.o Including wig consultations and wig fittings for Sylvester patients (locations may vary).• Provides patient education regarding cancer support services provided at Sylvester.• Patient Support:o Serving as a point of contact for patients to address their questions and concerns. Being in-person full-time.o Offering guidance and empathy throughout the cancer treatment journey.• Supports patients as a liaison to services and departments within Sylvester and UHEALTH.• Education and Information:o Providing information on available support services, including counseling, financial assistance, and community resources. Information folders (both paper and electronic).o Ensuring patients have access to educational materials and resources related to cancer and its treatments. Flyers, calendars (keeping all waiting areas up to date with information).• Care Coordination:o Coordinating appointments and ensuring timely access to medical specialists and treatments. Cancer Support Services Referral Work Queues My Wellness Check Scheduling for providers Multi-disciplinary Clinic coordinationo Collaborating with healthcare providers to facilitate a seamless patient experience.• Provides wayfinding services and tours to cancer patients when needed.• Communication and Advocacy:o Serving as an advocate for the patient, helping them communicate their needs to the healthcare team.o Facilitating communication between the patient and their healthcare providers.• Maintains confidentiality of patient information.• Performs patient call back to complete Patient Satisfaction Survey.• Assists with other clerical duties, not limited to faxing, scanning, copying, and ordering supplies.• Performs other duties as assigned.
