Registered Nurse Home Health

Job Summary:

Provides comprehensive home health nursing care to patients by coordinating services according to physician orders and agency protocols. Evaluates patient needs, administers treatments, and supervises home health aides and licensed practical/vocational nurses to ensure quality care delivery.

Responsibilities:

• 
  Coordinate and provide patient assessments and care in home health settings.
  
  
  
  
• 
  Supervise and evaluate home health aides and LPNs/LVNs, ensuring adherence to clinical standards.
  
  
  
  
• 
  Document patient progress, interventions, and outcomes accurately within the agencys EMR system.
  
  
  
  
• 
  Travel to patient homes within a specified radius, managing a caseload of 5-8 patients daily.
  
  
  
  
• 
  Participate in weekly team meetings and maintain compliance with agency policies and procedures.
  
  
  
  
• 
  Respond to on-call rotations and manage after-hours patient needs as assigned.
  
  
  
  
  
  
  
  
  Qualifications:
  
  
  
  
• 
  Hold an active, valid RN license in the state of Oregon.
  
  
  
  
• 
  Have a minimum of 1 year of recent adult home health or hospice nursing experience.
  
  
  
  
• 
  Proficient in the use of Electronic Medical Records, specifically HomeCare HomeBase (HCHB).
  
  
  
  
• 
  Demonstrate strong clinical skills in wound care, start of care, and follow-up visits.
  
  
  
  
• 
  Possess a reliable vehicle, valid drivers license, and current auto insurance.
  
  
  
  
• 
  Be willing to travel up to 75 miles from the home clinic.
