Key Responsibilities\n

• Conduct educational and therapeutic group sessions that empower patients and enhance their coping skills. Maintain accurate documentation of patient goals and responses to therapeutic activities.

• Participate actively in treatment-planning meetings, providing insights on patient progress and barriers to treatment. Update treatment plans with feedback from interdisciplinary team members and document all interactions meticulously.

• Identify and navigate resistance or conflict within client interactions—this includes addressing concerns with patients, their families, and other healthcare professionals. Employ conflict-resolution strategies to foster effective communication and facilitate positive patient outcomes.

• Develop, document, and manage comprehensive patient records in accordance with social work standards. This includes an accurate account of assessments, treatment plans, interventions, and closure documentation affecting the patient's journey.

• Craft personalized treatment plans that assess strengths, vulnerabilities, and specific psychosocial needs, allowing you to effectively allocate resources and interventions tailored to each individual.

You will work with a diverse patient population, many of whom face complex psychosocial challenges. Your efforts will significantly impact individuals and families who seek support for mental health issues, substance use disorders, and other biopsychosocial concerns. Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Current and active licensure as a Clinical Social Worker (LCSW) in the state of California.
