RN/LPN – Long-Term Care

We are seeking a dedicated and compassionate RN/LPN to join our long-term care team. If you are passionate about providing exceptional care to seniors and thrive in a collaborative environment, we would love to hear from you.
Position Summary
The nurse is responsible for assessing, planning, implementing, and evaluating resident care while ensuring compliance with healthcare regulations and facility policies. The nurse will work closely with residents, families, physicians, and interdisciplinary team members to promote the highest quality of life for our residents.
Responsibilities
Provide skilled nursing care to long-term care residents.
Assess residents' physical and emotional needs and develop individualized care plans.
Administer medications and treatments as prescribed.
Monitor resident conditions and communicate changes to physicians and family members.
Supervise and support Certified Nursing Assistants (CNAs).
Maintain accurate and timely documentation.
Ensure compliance with state, federal, and facility regulations.
Participate in quality improvement and infection control initiatives.
Qualifications
Current Registered Nurse RN/LPN license in good standing.
Long-term care or skilled nursing experience preferred.
Strong assessment, communication, and organizational skills.
Ability to work effectively in a team environment.
Commitment to providing compassionate, resident-centered care.
