Nurse Practitioner

Overview Sub‑Acute, Long‑Term Care is a sector of healthcare exploding with potential and work‑life balance flexibility (no on‑call or weekends). We are seeking a full‑time (40 hours per week) nurse practitioner for our Jersey Shore Skilled Nursing and Rehabilitation Center located in Jersey Shore, PA. The salary range for this position is $110,000.00 – $130,000.00 per year. Responsibilities The Advanced Practice Provider reports directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager and collaborates on‑site with the Center Leadership Team for center‑specific goals and priorities. Collaborates with the Medical Director and/or Attending Physician for patient‑specific medical care. Provides primary and urgent care services to nursing center residents in collaboration with other medical providers in the center. Works collaboratively with Center Leadership to enhance AlignMed Partners’ mission of improving lives through high‑quality health care and everyday compassion. Qualifications Graduate from an accredited School of Nursing program and licensed as a registered nurse. Certified as a Nurse Practitioner with a Master’s Degree in Nursing and specialization in Older Adult or Family NP (or other concentrations based on experience and business need). Minimum of two years experience working with adult or geriatric populations. In states where practice is allowed, an unrestricted professional license and DEA registration for each state in which they practice.
