Description
  • Conduct assessments of patients in either inpatient or outpatient settings and formulate personalized treatment plans
  • Deliver diverse physical therapy interventions targeting mobility, strength, balance, and daily functionality
  • Work closely with nurses, physicians, case managers, and fellow rehab specialists to enhance patient outcomes
  • Instruct patients and caregivers on therapeutic objectives, discharge processes, and home exercise regimens
  • Maintain accurate documentation of evaluations, treatments, and progress following facility and regulatory standards
  • Engage in care team discussions and interdisciplinary meetings to ensure cohesive patient care
Education
  • Doctorate in Physical Therapy (DPT) or an equivalent credential from a recognized program
Certifications
  • Currently licensed as a Physical Therapist in the state
  • CPR certification is preferred
  • Any additional specialized certifications (e.g., in geriatrics or neurology) would be advantageous but are not mandatory
