Description

• Conduct thorough evaluations of patients in both inpatient and outpatient settings, creating tailored treatment plans to meet their unique needs
• Implement physical therapy interventions that enhance mobility, build strength, and boost balance and overall function
• Engage collaboratively with a multidisciplinary team of nurses, physicians, and rehab professionals to maximize rehabilitation outcomes
• Guide patients and their families through therapeutic goals, discharge planning processes, and at-home exercise regimens
• Maintain precise documentation of evaluations, treatments, and patient progress to comply with facility protocols and regulatory standards
• Be an active participant in care team rounds and interdisciplinary meetings to ensure cohesive patient care

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist
• CPR certification preferred
• Specialized certifications in areas such as neurological or geriatric therapy are advantageous, though not mandatory

Skills

• Prior experience in a clinical setting preferred but not necessary
