Job Title: Licensed Master Social Worker (LMSW)

Location: Portland, OR
Duration: 13 Weeks
Schedule: Day Shift | 5x8-Hour Shifts (8:00 AM – 4:30 PM)

Compensation

Traveler Pay: $55–$60/hour ($2,200–$2,400/week) – Based on Experience
Local Pay: $45–$50/hour ($1,687–$1,875/week) – Based on Experience

Position Overview

We are seeking a dedicated Licensed Master Social Worker (LMSW) to provide social work services within a home health setting. The LMSW will support patients and families by assessing psychosocial needs, coordinating community resources, assisting with care planning, and promoting positive health outcomes. This position requires strong communication, organization, and independent decision-making skills while managing a home-based patient caseload.

Responsibilities

• Conduct comprehensive psychosocial assessments for patients receiving home health services.
• Develop and implement individualized care plans based on patient and family needs.
• Coordinate community resources, support services, and referrals to appropriate agencies.
• Assist patients and families with coping strategies, adjustment to illness, and care transitions.
• Collaborate with interdisciplinary healthcare teams to support continuity of care.
• Maintain accurate and timely documentation, including OASIS assessments and related records.
• Educate patients and caregivers regarding available support programs and services.
• Monitor patient progress and recommend interventions as needed.
• Ensure compliance with regulatory, agency, and documentation standards.

Assignment Details

• Day Shift: 8-Hour Shifts (Facility cannot accommodate 4x10s or 3x12s)
• Patient Ratio: 1:1
• Typical Caseload: Up to 6 patient visits per day
• Productivity Standard: 30 productivity points
• No weekend requirement
• No on-call requirement
• No floating between units
• Block scheduling not available
• Guaranteed Hours: Facility may call off up to one shift per pay period

Required Qualifications

• Minimum 1 year of Skilled Home Health Social Work experience required.
• OASIS experience required.
• Strong assessment, care coordination, and case management skills.
• Ability to work independently and manage a mobile patient caseload.
• Reliable transportation required.

Required Licensure & Certifications

• Active Oregon Social Work License required.
• Active Washington Social Work License required.
• BLS Certification required.
• Pending licensure may be considered; both Oregon and Washington licenses must be obtained prior to assignment start.
