Director of Social ServicesWolfeboro Bay Center – Wolfeboro, NH

Wolfeboro Bay Center is seeking a compassionate, experienced, and resident-focused Director of Social Services to join our leadership team.

This is an excellent opportunity for a social services professional who is passionate about advocating for residents, supporting families, and helping individuals navigate the challenges associated with short-term rehabilitation and long-term care.

As Director of Social Services, you will play an essential role in ensuring our residents' psychosocial, emotional, and discharge-planning needs are met while working closely with nursing, therapy, physicians, families, and the interdisciplinary team.

Salary

$60,000 – $80,000 per year , based on experience and qualifications.

What You'll Do

• Lead and oversee the Social Services Department

• Assess residents' psychosocial, emotional, and social needs

• Develop and implement individualized plans to address identified needs

• Advocate for residents and promote dignity, independence, choice, and quality of life

• Provide support and guidance to residents and families throughout their stay

• Participate in resident care planning and interdisciplinary team meetings

• Coordinate discharge planning and ensure safe, appropriate transitions of care

• Connect residents and families with community resources and services

• Assist residents and families with advance directives and other care-related planning

• Address resident and family concerns promptly and professionally

• Support residents experiencing adjustment difficulties, behavioral concerns, grief, or other psychosocial challenges

• Maintain accurate and timely documentation in accordance with facility, state, and federal requirements

• Participate in quality improvement and regulatory readiness initiatives

• Collaborate with nursing, therapy, admissions, activities, dietary, and other departments to provide comprehensive resident-centered care

Qualifications

• Previous social services experience in a skilled nursing, rehabilitation, long-term care, or healthcare setting strongly preferred

• Previous leadership or supervisory experience preferred

• Knowledge of federal and state long-term care regulations

• Strong understanding of discharge planning and community resources

• Excellent communication, interpersonal, and organizational skills

• Ability to effectively communicate with residents, families, healthcare professionals, and community partners

• Strong problem-solving and conflict-resolution abilities

• Compassionate, professional, and resident-centered approach

• Ability to manage multiple priorities in a fast-paced healthcare environment
