Description: We are seeking a full-time Physical Therapist for home health care. This role offers the opportunity to build meaningful relationships with patients. The average patient visit is approximately 45 minutes. You will have significant time to provide effective therapy. The position allows for broad independence while being part of a cohesive patient care team. Work-life balance and flexibility are key components of this role. The Physical Therapist will evaluate the patient's need for therapy and develop appropriate treatment plans. You will implement treatment programs in accordance with agency policies and professional standards of care. Effective communication with all health care team members is essential. You will document evaluations, treatment goals, and patient objectives. Educating patients and their family members on treatment rationale and procedures is also a key responsibility.
