Registered Nurse - Home Health

Job Description

Job Summary:

Seeking a dedicated Registered Nurse with recent home health experience to perform comprehensive patient assessments in a hybrid telehealth and in-field capacity. The role requires evaluation of health status, development of care plans, and coordination of services within the community.

Responsibilities:

• Conduct initial in-home pre-evaluation assessments of patients' health status, strengths, and care needs.
• Review and analyze medical documentation and referral forms relevant to each case.
• Input evaluation data into electronic forms and ensure accurate transmission.
• Document concerns, conflicting information, and issues encountered during assessments.
• Collaborate with consumers on healthcare options, supports, and waiver services.
• Facilitate communication between the Call Center and eligible consumers for enrollment into managed care plans.
• Maintain strong working knowledge of community resources, payor policies, and network services.
• Support care coordination by working effectively with interdisciplinary team members.
• Collect and record quality review data and case notes into CRM.
• Identify health promotion and illness prevention opportunities.
• Adhere to all performance standards and complete assigned duties as directed.

Qualifications:

• Current RN licensure in New York State.
• Minimum of two (2) years of recent home health assessment experience within the last fifteen (15) years.
• Proven ability to perform comprehensive in-home patient evaluations and assessments.
• Strong knowledge of state and agency regulations governing professional nursing practices.
• Excellent organizational, interpersonal, and communication skills, both written and verbal.
• Ability to work independently in a fast-paced, deadline-driven environment.
• Proficient with healthcare technology and electronic documentation systems.
