Position Overview Job Title: Licensed Practical Nurse (LPN) Location: Several Locations around Rochester Department: Clinical Reports To: Residence Manager Employment Type: Full-Time - 40 Hours, Monday - Friday, Days (7am - 3:30pm or 6am - 2:30pm) Starting Wage: $28.73-$32.83 per hour Why You Should Work for Lifetime Assistance? Your Core Responsibilities Deliver compassionate, person‑centered health care to individuals in residential settings. Administer medications and treatments as prescribed; monitor health status and report changes to the RN. Provide personal care, support daily living routines, and promote independence. Coordinate and accompany individuals to medical appointments; ensure follow‑up care is carried out. Assist the RN with developing nursing care plans, health‑related goals, and staff training. Respond to emergencies and behavioral incidents, utilizing SCIP‑R strategies. Act as shift leader when assigned and collaborate with interdisciplinary teams. Maintain accurate documentation in compliance with agency and regulatory requirements. Support a clean, safe, and welcoming home environment. What You Bring Education & Licensure: Current New York State Licensed Practical Nurse (LPN) license in good standing. Certifications: CPR and First Aid (or ability to obtain); SCIP‑R Level II certification or willingness to complete training. Requirements: Valid NYS driver’s license and ability to meet Lifetime Assistance’s Vehicle Operator Requirements. Experience & Skills: Strong communication, problem‑solving, and decision‑making skills; ability to follow medical directions and adapt to changing care needs. Physical Requirements: Ability to lift up to 35 pounds, respond to emergencies, and perform frequent physical activity.
