As a registered nurse with our Hospice program, you will provide compassionate and high‑quality care to individuals with terminal illnesses while supporting their loved ones. About the Role As a Registered Nurse with VNA Health Group's Hospice program, you will be responsible for providing skilled nursing care to patients in their place of residence, including homes, assisted living facilities, and skilled nursing facilities. You will work collaboratively with a multidisciplinary team to develop and implement individualized care plans that address the physical, emotional, and spiritual needs of our patients. This is a full‑time opportunity that offers a rewarding and challenging experience for a dedicated nurse. Responsibilities Assess patients' physical and emotional needs, develop and implement individualized care plans, and provide skilled nursing care in accordance with established standards and regulations. Collaborate with the interdisciplinary team to ensure comprehensive care, including physicians, social workers, home health aides, and spiritual care providers. Provide emotional support and bereavement counseling to patients and their families. Document patient care and interactions accurately and thoroughly in accordance with established policies and procedures. Participate in quality improvement initiatives and utilize data to drive improvements in patient care. In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $43.00 to USD $58.00.
