Key Responsibilities\n

As a Licensed Clinical Social Worker, you will:

• Conduct thorough psychosocial assessments of patients using interviews, observations, and detailed medical record reviews to understand the emotional and social factors affecting their recovery.

• Provide individual and family counseling, addressing emotional well-being and adjustment challenges, particularly as they relate to recovery and rehabilitation.

• Collaborate with the rehabilitation team, including physical and occupational therapists, to create comprehensive, individualized treatment plans that incorporate psychosocial needs.

• Facilitate the identification and fulfillment of patient needs for community resource access, such as support groups, vocational training, and housing assistance.

• Educate patients and their families about available community resources and aftercare services to support their transition post-rehabilitation.

• Become an integral part of discharge planning processes, ensuring smooth transitions to home or other care settings for patients.

Patient Population / Client Focus\n

You will primarily serve a diverse patient population recovering from various conditions such as substance use disorders, mobility impairments, neurological disorders, and other rehabilitation needs. Your understanding of adjustment issues, grief, and coping mechanisms will be vital as you help guide these individuals through their recovery journey, ultimately aiming for their successful reintegration into the community.

Qualifications\n

To qualify for this role, you must possess:

• A Master’s degree in Social Work from an accredited institution.
