Job Title: Case Manager RN Profession: Registered Nurse Specialty: Case Manager Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum 2 years License: Current RN License Certifications: BLS; Case Management Certification; BSN Must-Have: Current/recent experience in an Acute Care Adult Inpatient Hospital Setting Ability to float within the hospital based on census Availability for every other weekend commitment and holidays if applicable Description: The Case Manager RN is responsible for coordinating patient care and ensuring quality outcomes. The role requires the management of patients with medical, surgical, cardiac, and vascular diagnoses, along with some neuro cases. Candidates must demonstrate knowledge of admission criteria, concurrent review, continued stay reviews, appeals, and denials. Experience with care coordination and benefits eligibility is essential. Documentation experience in an EPIC system is required. Candidates must possess effective communication skills and the ability to work collaboratively within an interdisciplinary team. All candidates will be required to adhere to the policies and procedures of the facility. This position entails a commitment to high-quality patient care and efficient case management.
