Position Overview PeaceHealth Home Health in Florence, Oregon is seeking a Speech Pathologist for a Per Diem/Relief, 0.00 FTE, Day position. Salary range: $49.16 – $69.41 per hour, depending upon education, training, experience, and collective bargaining agreement. This role allows you to work in the patient’s own environment, engaging family and caregivers. About the Team Our multidisciplinary Home Health team consists of Physical Therapists, Occupational Therapists, Speech Language Pathologists, RN’s, Medical Social Workers, Registered Dietitians, and Bath Aides. The team meets regularly to collaborate and coordinate care, aiming for optimal outcomes. Job Summary Responsible for evaluating patients, determining plan-of-care and treatment goals, and participating in direct treatment within the scope of Speech and Language Pathology. Provides oversight to conditional licensees, fellows, students, therapy aides and volunteers, working as part of a patient‑care team. Responsibilities Evaluate assigned patients and establish treatment plans and goals, using data from patient records, interviews, observation, and team members. Complete patient treatment according to plan of care, modify it to meet patient needs, and update plans as appropriate. Document accurately and timely, meeting regulatory, licensure and professional standards. Collaborate with other healthcare professionals to ensure smooth and continuous care, acting as liaison and educational resource. Mentor and supervise new caregivers, conditional licensees, therapy aides, and students. Recommend and participate in the development of new services, quality‑improvement and monitoring activities. Participate in staff meetings, patient‑care meetings, in‑services, caregiver orientations, and other professional activities. Required Qualifications Education Master’s or Doctorate in Speech and Language Pathology accredited by the American Speech‑Language‑Hearing Association. Experience Requires 1 year of experience in Home Health. Credentials & Licensure Licensed Speech Pathologist – Oregon Certificate of Clinical Competence in Speech‑Language Pathology Basic Life Support through American Red Cross or American Heart Association (upon hire) Valid driver’s license and proof of insurance Benefits Benefits for positions less than 0.5 FTE include 403(b) retirement plan, wellness benefits, discount program, expanded Employee Assistance Program, and mental health program.
