As a Physical Therapist, you will play a crucial role in evaluating and addressing the physical therapy needs of patients. Your responsibilities will include:

Assessing, planning, treating, and evaluating patients to implement individualized physical therapy programs
Providing age and developmentally appropriate care
Interacting effectively with patients, doctors, and other healthcare team members
Completing timely documentation of patient care activities in the electronic medical record
Transferring and positioning patients safely

Requirements for this role include:

Need 1 year experience in setting
Valid driver's license and car insurance
COVID-19 vaccine (medical and religious exemptions may apply)
OASIS and start of care experience strongly preferred
BLS Certification
Active Illinois State License
