HOME HEALTH AIDE

Location:Beaumont, TX (Woodville, Spurger & Warren Territory)Job Summary:TheraCare Home Health is seeking a reliable and compassionate Home Health Aide (HHA) to provide high-quality personal care to patients in the home setting. In this role, you will support patients with daily living activities, promote a safe and comfortable environment, and work closely with the clinical team to ensure continuity of care. This is an ideal opportunity for someone who takes pride in patient care and wants to make a meaningful impact in the home health setting.Qualifications:Completion of a Certified Home Health Aide (HHA) ProgramMinimum of 1 year of experience as a Home Health Aide in a home health agencyStrong understanding of personal care, patient safety, and infection control practicesAbility to follow an established Plan of Care under RN supervisionExcellent communication skills with patients, families, and clinical staffDemonstrated compassion and commitment to the care of elderly and ill patientsAbility to accurately document care and report changes in patient conditionReliable transportation with valid driver’s license and current auto liability insuranceCurrent CPR certification and required health screeningsAbility to read, write, and communicate effectively in EnglishEssential Functions:Provide direct patient care under the supervision of a Registered Nurse in accordance with the Aide Plan of CareAssist patients with activities of daily living (ADLs) including bathing, grooming, dressing, and toiletingSupport safe patient transfers, ambulation, and positioningMonitor and report changes in patient condition to the supervising nurseMaintain a clean, safe, and comfortable home environment for patientsAssist with proper nutrition and hydration based on patient needsFollow and maintain infection control and safety protocolsComplete accurate and timely documentation in the medical recordCommunicate effectively with patients, families, and interdisciplinary team membersParticipate in interdisciplinary communication and case conferences as neededMaintain patient dignity, privacy, and confidentiality at all timesSupport efficient operations through strong time management and reliabilityAttend required in-services to maintain competency and compliancePromote agency policies, procedures, and commitment to quality careWe are an Equal opportunity employer; We offer an excellent benefit plan to include 401K with match, CEU reimbursement, vacation, sick, holidays, medical, dental, and supplemental insurance Plans as well as a Highly competitive compensation package.Diversity, Equity, and Inclusion are at the heart of Cantex.
