Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Ohio Physical Therapist license required to start the assignment.
• BLS certification required from AHA or ARC prior to start.
• Home Health experience required.
• Experience with HCHB (Homecare Homebase) documentation required.
• Ability to manage a weekly patient visit load consistent with Home Health expectations. (Client indicates typical weekly points: 32.)
• Valid driver's license and reliable transportation for travel within assigned territory (Perry and Morgan counties).
• Ability to work Monday through Friday, full-time, 40 hours per week.
• Preferred: previous travel or territory-based Home Health experience. Preferred items are not required.
