Job Title: Healthcare Consultant I - FLA - (HOMESTEAD/FLORIDA CITY) Address: Homestead, FL Job Description Coordinate case management activities for Medicaid Long Term Care/Comprehensive Program enrollees. Utilize critical thinking and judgment to ensure appropriate healthcare outcomes for members. Conduct comprehensive evaluations of members using various care management tools. Develop proactive courses of action to enhance members' overall wellness. Manage a caseload that includes supportive and medically complex members. Provide coaching and support to empower members in healthcare decision-making. Monitor and document care plans in compliance with regulatory guidelines. Critical Information Remote position; candidates must reside in Homestead or Florida City area (33033, 33032). Travel required approximately 75% of the time within the region for face-to-face visits. Work hours: Monday - Friday, 08:00 AM to 05:00 PM. Education/Licenses Needed Bachelor's degree in Social Work or related field is required. Verifyable High School Diploma or GED required.
