Job Title: PTA Profession: Therapy/Rehabilitation Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year Adult Home Health Experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Recent Home Health experience

Description: Therapy PTA providing home health services. Conduct home visits and periodic office meetings. Must have reliable transportation and a current driver's license. Agency-required liability insurance is necessary. Average number of patients per day is 5-6. Contractor will be assigned between 5-10 patients per day. Varied point values for different types of visits and activities. Documentation and travel hold different point values. No guaranteed hours; low patient census may lead to discussions regarding floating to nearby clinics. Types of cases include Start of Care and Follow Ups. Must be willing to travel up to 75 miles from the home clinic. Mileage will be reimbursed at the IRS standard rate. The commute to and from the first and last patient is not billable. Local applicants are accepted at the same rate. Must be open to traveling and potentially floating to other counties. Documentation must be completed within 24 hours of the appointment. Delayed or incomplete documentation affecting billing will result in cancellations. There are no guaranteed hours with this contract. Clinicians must hit weekly productivity points that equate to 40 hours per week. Declining patients may result in cancellations. Expectations to accept every patient on the schedule regardless of distance.
