Care Manager, Child & Family Services - NYC

Rising Ground in New York City seeks a Care Manager to coordinate comprehensive services for children and families across Manhattan, the Bronx, Brooklyn and Queens. You will oversee individualized care plans, facilitate access to Medicaid and other benefits, and ensure timely, accurate documentation. The role requires a BA/BS with two years’ experience, bilingual Spanish is a plus, strong written and verbal communication, and ability to travel within the catchment area. #J-18808-Ljbffr Rising Ground
