Position Summary We are seeking a full‑time, mission‑driven Family Physician to join our clinical team at Delta Health Center. The on‑site role is based in Mound Bayou, Mississippi and involves providing comprehensive primary care services from pediatrics to geriatrics. The Physician will diagnose and treat a broad range of acute and chronic conditions, address urgent care needs, and actively support community health initiatives. Prescribe medications, therapies, and referrals in accordance with clinic policy, formulary guidelines, and applicable Mississippi State regulations. Coordinate chronic disease management, developing individualized care plans and engaging care coordinators and specialists as needed. Deliver preventive health services, including immunizations, screenings, and counseling, and educate patients and families on lifestyle modifications. Participate in and support DHC’s community health programs, outreach events, and population health efforts. Collaborate within a multidisciplinary team—including nurses, mid‑level providers, behavioral health staff, and social workers—to ensure whole‑person, seamless patient care. Recognize conditions requiring specialist intervention and facilitate timely referrals and consultations. Maintain accurate, thorough, and timely electronic health records in compliance with FQHC, HIPAA, and clinical documentation standards. Requirements Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school. Completion of an ACGME‑accredited Family Medicine residency program.
