- Develop individualized care plans in collaboration with clients and their families to address specific goals, preferences, and challenges. \n
- Provide short-term and long-term counseling sessions, emotional support, and crisis intervention to clients grappling with substance use and mental health disorders. \n
- Advocate for the rights of clients, ensuring access to vital community resources, support services, and treatment programs. \n
- Collaborate with an interdisciplinary team, including healthcare professionals, counselors, and addiction specialists, to ensure cohesive and comprehensive care delivery. \n
- Regularly monitor clients' progress through virtual check-ins and assessments of their living conditions, addressing any safety concerns proactively. \n
- Facilitate family meetings to enhance communication and resolve conflicts among those involved in a client’s care. \n
- Educate clients and their families on available community resources, recovery support groups, and techniques for enhancing social engagement. \n
- Maintain accurate and comprehensive documentation regarding client assessments, care plans, progress notes, and any relevant interactions to ensure compliance with regulatory standards. \n

Your primary focus will be on clients struggling with substance use disorders and accompanying mental health challenges. Qualifications\n- \n
- Master's degree in Social Work from an accredited institution. \n
- Current and valid state licensure as a Licensed Clinical Social Worker (LCSW). \n
