Heart to Heart Hospice is looking for Circle of Care volunteers to make phone calls to hospice patients/caregivers and or family members to f/u with supply/medication refill needs and any questions or concerns that need to be addressed.

Volunteer reports any needs to the Volunteer Coordinator for f/u.
