Case Manager RN - Inpatient Pediatrics

Case Manager RN - Inpatient Pediatrics (260001UU) Responsible for ensuring patient progress and facilitating safe and sustainable transitions, collaborating with physicians and interdisciplinary teams to determine care plans, estimate length of stay, and plan discharge disposition. Establishes timelines, reviews medical records, participates in multidisciplinary rounds, escalates barriers, and works in partnership with physicians to consider alternative levels of care when needed. Facilitates communication among providers, documents avoidable care days, and communicates anticipated discharge dates. Coordinates discharge needs, collaborates with social work to manage patients with high psychosocial acuity, and ensures timely, accurate data entry. Provides hand‑off communication and directs patient assignments. Identifies and addresses unnecessary admissions, develops patient specific action plans for frequent utilizers, and documents conditions on admission. Technical Skills and Responsibilities Develop and coordinate Discharge Plan A and alternative Plan B, documenting in the medical record. Integrate patient story into the multidisciplinary plan of care. Coordinate access to services and interventions to achieve favorable outcomes within target LOS. Collaborate with physicians, nursing, social work, and other disciplines to eliminate barriers to efficient delivery of care. Use Physician Advisor per protocol for complex issues; determine next steps with physician. Lead or co‑lead Care Coordination Rounds and refer patients for Complex Care Rounds. Participate in clinical performance improvement activities. Build positive working relationships that advocate for the patient. Conduct team meetings for all unplanned readmissions within 30 days. Complete necessary paperwork to facilitate patient transitions across levels of care. Qualifications Graduate of an accredited School of Nursing; Associate’s Degree required. Licensed Registered Nurse in the State of Florida.
