Speech-Language Therapist (SLT) – PRNAscend Home Health & Hospice | Northern UtahAscend Home Health & Hospice is seeking a compassionate and motivated Speech-Language Therapist (SLT) to join our growing team on a PRN basis serving patients throughout Cache and Box Elder Counties in Northern Utah.This position offers flexibility and the opportunity to make a meaningful impact by helping patients improve communication, cognition, and swallowing abilities in the comfort of their own homes.Responsibilities:Evaluate and treat patients with speech, language, cognitive, voice, and swallowing disordersDevelop individualized treatment plans based on patient needs and goalsEducate patients, families, and caregivers on therapy techniques and care plansCollaborate with physicians, nurses, therapists, and the interdisciplinary care teamComplete timely and accurate documentationQualifications:Current Utah SLT license requiredHome health experience preferred, but not requiredStrong communication and organizational skillsCompassionate, dependable, and patient-focusedValid driver's license, reliable transportation, and current auto insurance requiredPosition Details:PRN / Flexible schedulingCoverage area includes Cache and Box Elder CountiesPay per visit DOE (dependent on experience)At Ascend, we are committed to providing compassionate, personalized care while supporting our team members with a positive and collaborative work environment.If you are passionate about helping patients improve quality of life and thrive at home, we would love to hear from you!Employment is contingent upon the results of a mandatory background screening and proof of licensure when applicable.
