Remote intake Specialist

Pay: $18.75 - $22.00 per hour Job description Schedule: Monday–Friday, 9:00AM-6:00PM Location: Remote or Hybrid Role Summary Grace Community Care & Homes is seeking a dependable and compassionate Intake Coordinator to manage new client intakes during after-hours. You’ll help families complete eligibility forms, verify Medicaid enrollment, and prepare cases for the day team to process. Responsibilities Answer calls, emails, and messages prospective clients and support coordinators Conduct scheduled Housing Supports and DDD intakes via phone Enter data into HMIS or intake tracking systems Verify Medicaid (NJ FamilyCare) coverage and MCO (Aetna, Horizon, Wellpoint, UHC, Fidelis) Prepare intake documentation for Prior Authorization and Billing teams Maintain confidentiality and ensure HIPAA compliance Qualifications Experience in healthcare, social services, or community-based programs preferred Knowledge of NJ Medicaid, DDD, or housing-related supports is a plus Strong communication and documentation skills Self-motivated, dependable, and able to work independently Tech-friendly (Therap, Acuity, Google Workspace) Prior experience in Medicaid, DDD, or human services intake strongly preferred Detail-oriented with excellent written and verbal communication Comfortable with data entry and client tracking systems (MS Office, HMIS, Electronic Records Systems) Able to maintain confidentiality and follow HIPAA guidelines High school diploma or associate’s degree minimum Job Type: Full-time Benefits 401(k) Health Care Plan (Medical, Dental & Vision) Retirement Plan (401k, IRA) Paid Time Off (Vacation, Sick & Public Holidays) #J-18808-Ljbffr Grace Community Care and Homes Inc.
