Community Skilled Health Care Centre is seeking skilled, compassionate, and dependable Licensed Practical Nurses (LPNs) to join our growing night shift team. Our mission is to provide patient-centered care while fostering a supportive, respectful, and team-driven environment.

Licensed Practical Nurse (LPN) Requirements:

• Active Ohio LPN license
• Long-term care experience preferred, but not required
• Strong clinical and communication skills
• Ability to work independently on night shift

Responsibilities of Licensed Practical Nurses (LPN):

• Administer medications and treatments as prescribed
• Monitor and document residents' conditions, including vital signs and changes in status
• Assist in developing and updating individualized care plans
• Provide direct nursing care including wound care and daily living support
• Collaborate with RNs, physicians, and interdisciplinary team members
• Ensure compliance with all facility and regulatory standards
• Respond to emergencies and provide appropriate care
• Supervise and support nursing assistants (STNAs/CNAs)
